变更网吧食品流通许可证所需材料 1、《食品流通变更许可申请书》; 2、原《食品流通许可证》正、copy; 3、变更事项的相关证明材料;illustrate：name、person in charge、主体类型改变，应先行变更营业执照，再办理食品流通许可证变更。同时涉及地址、许可范围变更的，应先行变更食品流通许可证，再变更营业执照。
