2. 工商营业执照副本或名称核准通知书（复印件）；
3. 经营场所（储存设施）产权或租赁证明，没有也不租赁储存场所从事批发业务的，应当出具本单位没有也不租赁储存场所的承诺书；
4. Main person in charge of the unit、安全生产管理人员任命文件及安全资格证，特种作业人员（危化品仓储保管员等）特种作业操作证（复印件）；
5. 设置安全管理机构和配备安全管理人员的文件；
6. 安全操作规程清单和安全管理制度（至少保护用品管理制度、安全教育培训制度、Hazardous chemicals purchase and sale management system、安全检查及隐患整改制度、事故报告处理制度、安全生产奖惩制度）；
7. 安全生产责任书（复印件）；
8. 事故应急救援预案；
9. 安全评价报告书及整改意见落实情况（经安评单位核实确认）；
10. 企业经营和储存场所平面图；
11. County (city)、区）安监局出具的初审意见和请示报告；
12. 经营属于监控化学品的危险化学品须出具国家、省有关主管部门的批准文件。
13. 经营许可证变更申请变更单位名称、economic type、单位负责人、经营地址（仅限于不存放危险化学品的）应《危险化学品经营许可证变更申请表》一式3份、电子版1份；
14. 原乙种危险化学品经营许可证副本；
15. 变更企业名称和变更企业类型的，须提交变更后的工商营业执照副本(a copy)；
16. 变更单位负责人的，须提交变更后单位负责人的安全资格证；
17. 变更经营地址的，须提交不存放危险化学品的承诺书、经营场所产权和租赁证明（复印件）。
18. 重新办理经营许可证已取证经营单位涉及改建、Expansion or relocation of operations、storage place，扩大许可经营范围应重新申请办理经营许可证，原乙种危险化学品经营许可证副本；
19. 首次办理经营许可证需提交的申报资料。
20. 到期换证
21. 《危险化学品经营许可证换证申请表》一式3份、电子版1份；
22. 原乙种危险化学品经营许可证副本；
23. 工商营业执照副本（复印件）；
24. 经营场所（储存设施）产权或租赁证明；
25. Main person in charge of the unit、安全生产管理人员任命文件及安全资格证，特种作业人员（危化品仓储保管员等）特种作业操作证（复印件）；
26. 安全生产责任书（复印件）；
27. Safety evaluation report；
28. 属委托审查的单位还需要提供区、县（市）安监局审查合格并盖章的《乙种危险化学品经营许可证审查表》。
