Application conditions
药品经营许可证分两类：一种是零售连锁药店，此类药品经营许可证如今可以继续办理。
零售型药品经营许可申请开办药品零售（连锁）企业的向市药品监督局、申请开办药品零售企业的向拟办企业所在地药品监督分局提出筹建申请（开办零售（连锁）企业的，需说明连锁的管理体制），and submit the following application materials：1．药品、医疗器械经营许可证管理系统企业端“企业筹建申请”（3.5寸盘）（申请人在网站下载企业端程序并正确填报、导出）；2．拟办企业法定代表人、企业负责人的学历证明、执业资格或职称证明原件、Copy and resume；3．拟经营药品的类别和范围；4．拟设营业场所、Warehousing facilities、设备情况（附拟注册地址的地理位置图、Floor plan (specify area)，并注明与药品零售企业之间的最短可行进距离）；拟设仓库地理位置图、平面图（注明面积、长宽高）；5．开办零售（连锁）企业，还应提交所属各门店《药品经营许可证》、《营业执照》复印件以及资产相关证明；6．申请材料真实性的自我保证声明，并对材料作出如有虚假承担法律责任的承诺；如需提交软盘的，一并作出如有病毒引起数据文件丢失自行负责的承诺；7．凡申请企业申报材料时，申请人不是法定代表人或负责人本人，企业应当提交《授权委托书》2份；8．按申请材料顺序制作目录。
另外一种就是药品批发公司用的药品经营许可证，这样的药品经营许可证目前几乎已经无法办理。对药品批发公司的硬件和软件都是很严格的
