2. Medical institution architectural design plan；
3. List of legal representatives or principals of medical institutions and heads of departments and relevant qualification certificates、Practice certificate, etc. (including physician、Pharmacist、Nurse certificate；Practicing qualification or professional technical qualification certificate and "Shenzhen Pharmaceutical Industry Practitioner Position Certificate" or registration certificate at the Guangdong Provincial Licensed Pharmacist Registration Center, etc.)；
4. Secondary fire protection：Fire protection opinions issued by the fire department regarding buildings used in medical institutions ，"Fire Safety Inspection Opinion" or fire protection filing receipt, etc.；
5. Disposal of medical waste (disposal contract)；After the outpatient department opens，It is also necessary to submit the "Medical Waste Disposal Agreement" issued by the environmental protection department；
6. Disposal of disinfected items：For example, if the outpatient department has a detoxification supply room,，Need to submit expert’s acceptance status；If you entrust other disinfection supply organizations，A commissioned disinfection agreement needs to be submitted；
7. Outpatient department with X-ray room，The inspection and acceptance status of the X-ray room needs to be submitted。

而从7月1日起，龙岗区继续“扩大战线”，将其管辖范围内的社会办医疗机构也纳入“叫停”范围，具体包括三类：① 龙岗区各公立医院（含一级医院），除急诊科外，全面停止普通门诊静脉输液服务。② 中医馆、中医类诊所、infirmary，自2025年7月1日起禁止进行输液治疗。

③ 社区健康服务中心、outpatient department、西医类诊所，2025年7月1日起原则上禁止对53种常见病多发病进行输液治疗。
