《保安服务公司设立申请表》。

legal representative、investor、主要管理人员有效身份证件、个人简历（高中以上至今的简历情况及家庭成员情况）；公司组成人员不少于3人；出资人是法人的须提供营业执照复印件。
legal representative、主要管理人员大专以上学历复印件，并提供学历认定。
legal representative、主要管理人员保安师资格证书复印件。
legal representative、主要管理人员应具有5年以上军队、public security、Safety、trial、Prosecution、Judicial administration or public security、Security operation and management work experience：
军队工作经验：在军队、武警部队服兵役5年以上的军官（仕官、士兵工作经验不予受理），提供军官证、复转军人证等复印件；
政法单位工作经验：public security、Safety、trial、Prosecution、司法行政部门任职5年以上的国家公务员，提供原工作证（警官证、法官证等）、Copy of retirement certificate。

退休或离职的国家公务员（辞去公职或者退休的，原系领导成员的公务员在离职三年内，其他公务员在离职两年内的不予受理）；

治安保卫工作经验：县级以上人民政府确定的治安保卫重点单位从事治安保卫工作5年以上的保卫处、department、科长等内保干部，并提供在该单位5年以上社保部门打印的社保交费证明；
保安管理工作经验：在依法设立的保安服务公司和保安培训学校中担任副总经理职务以上和保安培训学校任法定代表人的人员（保安公司部门的部长、队长工作经验不符合

），提供该保安服务公司的《保安服务许可证》《营业执照》复印件（复印件需要盖公章）,和在该公司5年以上社保部门打印的社保交费证明。

公司办公住所有效证明文件（办公住所应是本市的，属于商业性质，且面积不少于200平方米）。

申请人租赁的，提供租赁合同（租赁期自申报之日起不少于2年），产权证或购房协议、业主有效身份证或租赁单位营业执照复印件。
办公住所平面图及照片（照片应有体现保安公司的标志、标识，上墙的主要规章制度等内容）。
提供保安服务所需的有关设施设备、交通工具等证明材料（附清单及照片）。
具备电脑、fixed、办公桌（椅）、fax machine、打印机和交通工具（其中汽车不少于2辆）等办公设施设备等有效证明材料。
拟从事安全技术防范报警运营服务的，办公场所应设置监控中心，配备不少于3辆的巡逻车辆；具有专业工程人员，提供报警运营服务所需的设施设备等有效证明材料，且公司组成人员不得少于5人。
拟从事人力范围保安服务的，须具备车辆、通信设施、防卫器械等设施设备的有效证明材料。
