三、需要提供的资料： 1、法定代表人或负责人的身份证原件及复印件； 2、企业营业执照原件及复印件 3、法人或负责人的健康证（D类）原件及复印件； 4、经营场所房产证复印件（加盖权利人公章）、租赁协议原件及复印件； 5、食品安全管理人员、食品安全专业技术人员的聘用合同文件及身份证原件、复印件（由法人或负责人担任可不用提供）； 6、经营场所空间布局平面图；（标明设备设施的位置、面积、用途等） 7、供货、销售流程文件； 8、与食品经营相适应的设备、工具清单； 9、特殊食品含保健食品销售的还需提供保健食品生产厂家的营业执照、生产许可证、产品检测报告、产品批准证书（加盖生产厂家公章）。
