1. 企业法人营业执照副本复印件；
2. 房屋租赁协议及出租方房屋产权证明复印件；
3. SP年检单位是一年内新设立的公司需提交会计师事务所出具的验资报告原件；
4. SP年检单位章程（加盖工商局档案查询章原件）；
5. 股东为自然人，需提交其身份证复印件；
6. 主机托管协议书复印件及IDC证书复印件；
7. SP年检单位法人、股东的身份证复印件、学历证明/简历复印件；
8. 总公司组织机构代码证副本复印件；
