10. 三名以上专职演出经纪人员的资格证明 (Performance agent qualification certificate issued by China Performance Industry Association)Original、复印件及身份证复印件；
11. 资金证明（复印件盖公司公章）；
12. 开户许可证原件复印件（复印件盖公司公章）；
